The Extreme Makeover franchise gets another spin-off, with this version following eight obese people who set out to lose half their body weight. The series documents the major physical changes each participant goes through over the course of their transformation.

In each episode, one participant's journey will be the focus as they show a year's worth of diet and exercise over the course of one hour. Trainer Chris Powell has been enlisted to coach them all through the process.
